    Default Health declaration booklet

    Well I have never been so excited to pick up a piece of paper from my doctors! It's taken 10 weeks to get my Health declaration booklet filled in so I want to get it sent off asap.
    There's no address to post it to! There was no "pre paid envelope" that the instructions refer to.
    Please help!

    Im sorry i dont know the address. If you do a search on the forum the address has been given out recently. Either that or try their phone line. It might be an answer they will actually give ;-)

    Thanks, I did and I found a thread from Jan saying its the Manchester address so I'm off to post it now
