Hi I have a new starter coming soon and wondered what you all use to gather information from parents that is written down. I used to use a booklet which had various questions for parents to fill in but dont have it anymore. Does anyone know where I can get a simple a4 sheet with basic questions for parents to fill in. I know I could make my own but am a bit pushed for time!!
