Question about EY2 form??

Do I have to download and print out all 26 pages or will ofsted send you a form if you ring up.
My daughter will be 16 in April, do I fill out the form before or after her birthday?
I'm sure this has been asked before but I couldn't find it so will be grateful if anyone knows,thanks

I filled it in online and printed the signature page for my friend to sign then scanned it in and emailed both bits to ofsted

Once your daughter is 16 Ofsted will contact you and send the necessary paperwork for you to complete and return (you dont need to do anything before her birthday)

once they recieved the email the CRB form arrived just 2 days later

I received the one for my son 2 days after his birthday complete with CRB form and everything. Didn't ring and remind Ofsted it just arrived. That was last Oct.
