    Default Care plans, scotland

Does anyone have a rough template on a care plan they use in Scotland? I'm unsure what info it actually has to have?

That's what I'd always thought it was so have always done my own all about me forms when a child starts. I've recently had training where they mentioned having care plans in place for children and having things like milestones, next steps etc on them?
