    Hi all,

    How do you all gather opinions about your service etc from parents, kids and other professionals and how and where do you document it?

    I use a questionnaire for parents.
    Somewhere in the depths of ofsted publications, there is a 20 question questionnaire for children's feedback that I use

    For other settings or professionals, I either jot down the date, time, location and persons name along with whatever comment was made on a post it note, photocopy any feedback that is in written form or print off a text message.

    These are then stored in a clear wallet labelled 'Feedback' in my CM folder as physical evidence and also mentioned in my SEF

    I have a questionnaire - I ask different questions to get a range of answers.

    I tend to find our parents don't like writing much they are better when we ask them verbally
