    Default Accident incident forms

    Hi all

    Filling in one if these forms is a little war and peace - which is fine,but then collect lo from nursery and I an given a postage stamp sized accident firm (ok maybe a bit bigger..) with name accident time date. That's it!?!?!

    Can we use this format - anyone know where I can get some pls!!

    I'm not really sure tbh. But I prefer the way I do it with full descriptions and body map showing where the injury was etc. I feel it safeguards myself more as I have a really accurate account of what happened

    That's more than I got with 4 year old in school nursery class - TA said as he came out there was a bit of an incident some pushing and X fell and that's how he got the big bump in middle of forehead!

    Filled in an existing injury form and got Mum to sign with TA name and what was said.

    I prefer ours too

    Believe me, the CMing accident/incident forms are a doddle.

    You should see the ones I used to have to fill out when I worked on the railways: dozens upon dozens of questions/fields/dropdown lists/tick-boxes (12 pages when printed out) for someone who's maybe dumb enough to fall over their own sports bag.
