    Default adhoc booking form?

does anyone have one? do you just write in the days/hours they will do apon booking and get parents to sign or is there more I need to put on it? I dont currently have one but it was pointed out to me that they could change their minds and I would have nothing to prove they said they would come on certain days. parents do pay in advance but somtimes thats on the morning that they are comming.

I'll be watching with interest as I think it's time to change my ad- hoc procedures. I had a parent cancel for today at 8:30 this morning after I was up & ready for the child to arrive at 7:00. AND they expect not to pay. So, I think it's time for an overhaul. I'm planning on doing a contract review over easter and demanding payment in full at time of booking - not on the day.
