ncma ones are out of stock anyway although it wont tell you on the site, I ordered one beginning of Sept and they have said it will be end of oct before I recieve it, they only told me this after chasing up with emails.

I need to make up something in the meantime as hoping for pre-reg inspection before end of october.

You can make your own as long as the parent can be given a copy, it is dated, the childs name, description of what happened and any action taken such as cold compress applied etc. Then you and the parent has to sign it and it has to be clear that it is the parents signature so I label mine "parent Signature and Childminder signature.
