Hi there, was told I needed maintenace records for equipment, premises and vehicles, can anyone help with this or have a copy of a templete.

I would think it just means keep the records you are given everytime you have some maintenance done. For example if you have your heating boiler serviced keep the certificate they give you, when you car is MOT'd or serviced keep the papework they give you to demonstrate that you keep it maintained. Its just so that you can demonstrate you keep things in good working order.

As you are in Scotland the requirements are slightly different from England.

I believe Rosebud is correct ( I am doing self assessment just now) and I am scanning in gas boiler check/cooker, car mot, smoke alarm install certificate.
