Bruises/marks before arrival

My little one I mind often has a little bruise on her head, she is one and learning to walk same as my daughter so I know these are just normal bumps.

Do I have to document them and get a signature from parent? I have a feeling I do but where do I note this? In my accident book?

They need to be recorded as existing injuries - details of the injury and parent needs to sign and date
