Would anybody mind sharing the paperwork they use when using a childminding assistant please?

I'd really appreciate it too please, my assistant is my mum, but my inspector said I'll need to get all the paperwork for her as if she wasnt my mum. Its on my pretty long to do list so would be a great help.
