I have put together a summative assessment for a new mindee starter at nursery school this next term in an effort to work in partnership with them.

Could you kindly have a look and see if it looks OK to you or if something is badly put or needs changing or is in inappropriate for the purpose. (Wil not send it to nursery obv. without mother's permission.)
cheers - here it is:

Summative Assessment

Child's Name: *******
Date: 30/12/11

In the last 3 months We have been working on:
• Independence in choosing and carrying out activities without needing support all the time. (PSED)
• Controlling frustration and expressing feelings appropriately and asking for things politely (PSED)
• IT skills and puzzles, numbers and letters (CLL/PSRN/KUW)
• Independence in toileting
• Swimming and physical confidence on playground equipment (PD)
• Sharing toys and the minder’s attention/time. (PSED)
• Art and creative expression (painting, music) (CD)s
• Small world play and role play. (trains, cars, spiderman, batman) (ALL)

We have achieved:
• A greater degree of self control although xx continues to need support and encouragement with sharing and with anger management.
• Great advances in number and letter recognition and IT skills
• Improving in controlled mark making and beginning writing
• Increasing confidence in the swimming pool and playpark.
• Can usually manage the toilet alone but needs assistance with wiping.
• Socialising skills are improving. He tends to play with older children (at soft play sessions or in the playground) and is less interested in his own age or younger, although he will express concern if they are upset and will try to help them.
• He has begun to help care for our animals and handle them appropriately and with affection. Likewise our cat and dog.

Parents comments on achievements:

Mother reports that he has improved with number skills, that he shows concern and interest in other mindees, that he enjoys swimming and his time at the child minding setting. He is looking forward to nursery and is confident and self assured about going. His food choices are gradually widening. His behaviour has recently improved somewhat but he needs constant guidelines and reminders of changes

In the next 3 months we will be looking to achieve:

• Increased socialisation and cooperative behaviour
• Growing confidence in swimming and his physical abilities
• Continue and develop his interest in numbers, letters and books
• Stimulate more creative development with art, crafts and music
• Encourage more caring behaviour towards our pets and our other mindees

Working in partnerships with parents.
Parents give me verbal reports which goes into his daily diary.

Working in partnership with other settings.
Hoping to work in partnership with Nursery.

Hi hon, good report.

I would maybe just restructure it slightly. You could still include the 'we've been working on' section but what is more important is what he has achieved so the second section.

I would use the achievement, followed by EYFS category (PSED) sub category (i.e. dispositions and attitudes) or an example followed by a mini evaluation.

i.e. xxx can now manage his emotions with support. PSED, self control, 30-50 months.

or the other day xxxx did not get upset when xx took his toy instead coming to me for help. PSED, self control, 30-50m. This shows he is starting to manage his emotions.

Does that make sense??

I would then link his next steps specifically to EYFS so linking to what you have listed you might put

PSED

to support him to manage his behaviour more indepently
- then list some of the PSED things you are going to work on

PD

support him to develop in movement and bodily awareness through encouragement
- swimming activities etc.

hope that helps :)

Hi, your summative report looks good and has plenty of information in it - we all do things a little differently and they're all correct, for example in my area we use a standardised one that the Council issue. These have all the information as you have listed but linked to age in months so that we can plot the development of the child and see where a child will be ahead or behind in months. This way we can see areas to focus on in our individual planning x

Thanks funnymnx. I wonder if you could put an attachment of this council sheet you are issued? Might help us newbies. The dev grids are still a really thorny point for me
