Has anyone used these before? Are they suitable for just one-off attendances? I'm assuming they are a shorter version of the full contracts and that you still have to record the child's personal details as you would with full-timers?

I have not personally used them - but have seen them and yes a shorten version and yes do need normal details.

Don'y forget if a one off - you need to cover the cost of the paperwork.

I have just filled one in today!! Ex mindee coming for the afternoon so mum can go to an interview.

I have added my Consent form with permissions and agreeing to all my Policies. The parent in this case already has a copy. Also I do a child information form.

My normal rate is £5ph I have charged £7ph for the 3 hours this afternoon which more than covers the cost of paperwork.
