Help! Examples needed

Hi there,

I am curretly in the process of registering and am now at the Risk Assesment stage. Trouble is I have never done one in my life. I was wondering if one of you lovelies would be happy to email me an example of one of yours? I am not looking to copy it just use it as a guide as to how to set one out.
