disclosures

I have sent away the registration paperwork last mth, and i have today recieved disclosure papers, for myself and husband,
i told the lady i just did one little then two mths ago as i was working in a nursery, so dont know what to put , and dont know what forms are for who, the police act disclosure for myself or application to join PVG Scheme,
Also do u pay for it?? as the lady who issued it has scored out the part where u enter card details,
just a tad confused, and any help would be really appreciated

We were told in our meeing that even if we had one we had to get another but dont need to pay. (I am in Wales )
