Potty training diary

I am potty training 2yo mindee alongside my own DD and wondered if anyone knows of any free printable potty training diaries? (to share information between home and setting, have got charts already) I've had a google and can't find any, really should stop being lazy and make my own but if anyone knows of one please can you let me know.
