URGENT - Accident form please

Hi there

I have just looked and have run out! One of my parents has sent me a text to say their LO has a small patch of sunburn around his neck where I have obviously missed the suncream and so i need one for drop of first thing, does anyone have one they could share with me please?

i would make one on Word hon. Mine just has child's name and date, details of accident, what treatment i gave (if any) advice for parent, signature space for me and parent.:thumbsup:
