Contract ammendment form

I need to devise a contract ammendment form for one of my parents, my mind has gone completely blank as to what to put on it :blush:

Can anyone point me in the right direction please?

I just did one for revised hours, mindee started school. I titled it "contract ammendment". Put on the childs name, new hours for term time and school holidays, parent signature and date, childminder signature and date. Hope I got it right :blush: Kept one myself and gave one to parent.

I drew one up this morning on a letterhead rather than wasting a whole contract - it says:

Contract Amendment 1

From (date) the contracted hours for (child name) will run from (start time) to (finish time) on (days) (except Bank Holidays unless otherwise specified).

These hours will be charged at the standard rate of £3.00 per hour.

No other aspect of the contract shall change. All other terms and conditions of the original contract shall apply as normal, and this amendment of hours constitutes a legal and binding agreement between Laura Doran Childminding and (Parent name).

Both parties have received a copy of this amendment.
