Parent questionnaire

Hi all

My lo's have been with me for 4 weeks now so I was thinking about doing a questionnaire for them including questions such as:-

Are you happy with the care xxx recieves?
Are there any activities you would like me to consider adding to xxx's routine?
How do you feel xxx has settled in?
Do you have any concerns about xxx at present?
(just examples)

What other things would you suggest to add? Mind is blank(still asleep!)
