risk assesments

I have just sent the local council a planning aplication for a 2 storey extension. Ofsted said I need to send them detailed risk asesment and photos, does any one have a template for this, or know how I should set it out and what to include.

Did ofsted want to see your plans as well? Am thinking of converting a room but no planning permission is needed just building regulations ( we were thinking about forgetting to apply for building regs:) ) would ofsted need to see the building regulations submission?

no all she asked for were risk assesment and photos, so a risk assesment of the on goinging works than a another for the finished rooms.
