Ending Contract

I am about to give notice with a parent (mutual agreement).

I know somewhere on the forum I have seen some sort of paperwork to advise that the contract ended on a certain date and that it is the parents responsibility to contact Tax Credits etc.

I can't seem to get my wording right......! Can anyone help..........please
