parent info pack

I am thinking about making a parent info pack for them to fill in about there child. what would you think i would need to kow apart from the obvious
ie
name
dob
address/ phone numbers
emergancy contact
health req
food /diety needs
religion

permision slips

You might also like to ask about favourite toys, likes and dislikes. Also maybe general routines like sleeping etc. Hope that helps Hx
