Childrens questionairre

Hi all, have given a questionairre out to parents but am trying to find one to give the children to fill in, they are quite keen after seeing one given to parents.

Hope you having a good half term, we are off to cinema to see Despicable me 3D this afternoon.

I have seen one somewhere with smiley faces for children but can't remember where! Will take a look and get back to you. Our children's centre gave out questionnaires to the children last weel asking if they liked it, if they enjoyed the activity and if they wanted to come again. The children were all aged 1-2 so not easy to get them to comply!!

Thanks Sarah, have made a simpler one like that for some of the younger children and using the Ofsted one for the older ones.
