crb form

I.ve printed the ey2 form from ofsted's site to fill in ,but from what i know this form is for my husband only,which form do i need to complete?Is it the same ey 2?i need some help with this,please

Do you mean the crb form? These are sent to you after you send your application form to ofsted. Or if for an assisstant form they are sent after you send the form back to ofsted.
