View Full Version : Help! how do I document a bite!

Mindee got badly bitten by a child at soft play this morning but I've gone and run out of incident forms.

Obviously I need to record this somewhere - what shall I do? Would just a handwritten piece of paper suffice?

I would have thought that as long as all the info required is is present it will be fine on a piece of paper. Sure some1 else will be along who knows for sure xx

I did that before, wrote it on a bit of paper, made sure all the info thats required on the forms was on it and got mum to sign. Popped it in childs file.
Mrs O did see it and read it and didnt make any comment

I agree it's the information that matters not what it's written on.

I often grab a piece of A4 paper and write it down.

Date... time... names... witnesses if appropriate... what happened... action taken... first aid...

I hope mindee is ok xx

Yes the manager recorded it for her records! She was very good actually and even talked to the parent concerned.

Yes she is fine now - her mother was a bit shocked, she was bitten on both forearms and has quite nasty bruises. Mum was ok though she realises that its just one of those things some kids go through.

I was right by her with my 2 other mindees but the little boy was soooo fast I couldn't get to her quick enough.

By the time I had got off the play equipment he had bitten someone else :eek:
