Could I be really cheeky and ask if I can have a copy of somebodies forms. I have ofsted due soon and they have told me that my forms do not have enough info on them:rolleyes:

Are you an NCMA member? You can get them from the shop.

I use the NCMA ones as they are accurate and in duplicate.

Sorry can't help but I think that this is one area where I needed to get it right.

As above I have always used the NCMA books, now folder.

I even used them for my Cub Pack they are so clear and I like the idea that the parents get a copy.

Same here use NCMA ones, they are duplicating so have a copy for the parents.

I think you can make them up easily enough if you really want. You need details of accident/incident, what the injury was, what you did, how child was, info/advice for parents on treating/monitoring injury and a column for you and parent to sign.

Some of the bought ones have a picture of a child so you can 'map' the injuries if any.
