End of contract comments

Hi,

Two of my mindees are off to school in Sept so their contracts are ending. Does any one have comment sheet they wouldn't mind sharing that I can give to parents so they can give their views on the standard care their children have received etc in the last 4 years. Or any ideas for questions I can put on the form.
