yes, I've done it. it's a requirement by the local authority but ours have passed it on to us directly (in a nursery the managers would fill it in for everyone).

Sarah, your early years team should've contacted you about it - that's terrible but not surprising given some of the d.o.'s:panic: there is a cut off date for completion

Trying to fill the survey in now.

What do you put for practitioner's role as none of the roles fit childminders?

Manager / Deputy Manager Manager or Deputy Manager responsible for the Practitioners within the setting

Senior early years worker Supervise staff working in a specific area but not directly assigned to a room. This includes staff with specialisms within their role, such as SENCO, Early Years Professional, Qualified Teacher

Room leader Directly responsible for staff and children in an assigned room.

Early years worker Supervise children but monitored by senior members of staff.

Admin / Finance / Facility Person No direct involvement with children in the setting

And do we have an ISA number, because if we have I don't know what mine is!!

No option for childminder which really annoys me.

We're not a manager either as it says "responsible for the practitioners within the setting. No where does it say sole responsibility of the children.

Perhaps I will try and not put a job title.

that's what I must have done (I can't go back and retrace my steps) because no job title is showing on my record)
