Parents questionnaire, what to ask?

Hi all, i am wanting to provide my mindees parents with a questionnaire, something i can give them every 6 months.

Anyone any ideas of questions i should ask.

if there happy with your services. is there anythink they would like to see different. do they like menu.would you like a weekly menu plan. do they understand eyfs, are they happy with the feedback you give and the way its given ie email phone,paper, is your child progressing.have you any worries? sorry cant think what else i write without looking.hope this has given you an idea.

This is mine I use. It is mainly Bromleys addapted for me. Feel free to use it if it is any good.
