I have been asked a lot about this subject recently in various pms so I thought it might be useful to put something together to share.
Planning for an individual child's interests is about thinking what the child enjoys (through your observations and assessments) and coming up with ways to follow his interests that will excite and interest him.
The Eyfs clearly says that play is a balance of adult-led and child initiated activities.
By writing this kind of planning you are taking the child's interest and building on it...
You might find it useful to keep for other children, so you do not necessarily need to put the child's name on it... just keep it general and put it in a file somewhere for the future.
Here is an example planning sheet, with a blank one attached so you can print it off and use it if it suits your way of doing things...
