Advice needed with writing an Incident report

I'm looking for some advice on how to write do an incident report. I have written a report similar to an accident report but not sure if I name the children involved in the report or if it needs to be kept confidential

You should name the child about whom the incident is written.

Any other children should not be named for confidentiality.

Yes I know they will go to parents and say xx happened, but from a paperwork point of view you must not name them.

Just keep it short and factual (I waffle too much) and if you need to name other persons use initials
