have you been on the Bromley site they have some example planning sheets and as for your winter planning have a look at activity ideas under January there are loads of great ideas there.

Hope that helps you xxx

On the Eyfs CD rom there are some good planning examples including a really helpful continuous provision planning sheet which has a blank one and an example one filled out. All printable and amendable too!

I know it is a monthly sheet you are after, but if a weekly one works for you then I have found misstinkerbell's one fantastic. It covers everything I need to do, and I fill parts in in advance and parts as they happen and parts at end of day. It is perfect for my needs and you might be able to find a way to make it work monthly if you wanted.
This is a link to the thread she posted it on (hope this is okay misstinkerbell but your sheet is brilliant) http://childmindinghelp.co.uk/forum/showthread.php?t=43021&highlight=planning+sheet
