Question about filling in CRB form for assistant

My daughter has been sent a crb form to register as my assistant she rang to ask me does she have to fill in the back page y and x

she then read infro form saying an ofsted person must do that bit so how do they see her proofs of id passport ect?

Does she leave all this blank or send in passport number or what they want?

When i done mine they came her to check the details then took the form away

Can anyone remember what their assistants done please

You are a registered person so you can fill in the bits at the back with passport number, drivers licence etc.

Then you both sign it (her on the inside, you under the bit you've filled in) and send it off.

I did my dd's a few weeks ago and rang them to check first :D
