leaving appraisals

Hi one of my mindees is leaving on thursday and I would like the parent to do a last appraisal of care,what sort of questions do you put on these forms please

I just normally ask for a reference when they are leaving.

I do have a general questionnaire for how they like my service, is that what you mean?
