I am trying to fill in the assessment for each child, im not really sure what to write as evidence for each area how do we show the evidence for each area??

Not sure what you are doing??
You need to use the Eyfs guidance to see what age and stage the child is working within...
then you can spot which section your obs fit into (that's the assessment part of the whole thing)... checking how the child is working within the Eyfs.
Then you need to think about the child's next steps... again the Eyfs guidance will help you.
