Recording Assistant Hours

I have to design a form to show assistants hours and pay. Does anyone have anything they can share before I put my thinking cap on?

i dont have the pay part as she's down as voluntary (doing nvq 3 and my sister etc) i just use the daily time sheets from ncma for her hours
