Do I need a doctor prescription form to be able to give this or could a parent form be enough like calpol etc?

It is for hay fever

I have this for a mindee who has a mild egg allergy. It is an over the counter medicine which does not require a prescription from a doctor.

I have used the medicine section in my NCMA accident book and just got mum to put name of medicine reason for use and sign :thumbsup:

Just to add: Obviously you have to get her to sign if you ever give them any!

Same here - I used it when little one had chicken pox. They forgot to send some one day and I text dad to get permission to use my own and then filled in medical chart getting mum to sign on collection.

i havent got a mindee who is using it but have one taking a tablet form.

last summer i did what others have already said, filled in an ncma medication form and parents signed it giving consent and then to see that I have given it.
