Most of the ra's I've seen are done in a table format but the other day I saw one that was just a list of things to check in the toddler group ie. stair gate across stairs, check outside area before letting children out to play etc...

To me a list just seems much more clear and all you need to do is access it regularly and just change the date to say it's been reviewed? What do you think?

Also how often do you do a r/a. Once you've done one for a park you regularly go to you don't need to do much more do you?

I think you should just do what suits you. I have a couple of different sorts, a tick list for my daily house/garden checks, which helps to prompt me. Then I have full written ones, I do a genertic one (which I review reguarly) for the park, walking to school, going in the car, drop-in's etc, and then I do one for each new outing we go on or for anything not already covered.

I do find myself adding a couple a month, more in the holidays
