Im about to take on twins and am getting together all my forms ready for their parents......

So far i have:-

consent form for taking photos
Parental responsibility
parental permission for observations
record of child's individual needs
consent form covering: outings, large play equipment, car/ car seats,transport in private vehicle, public transport,children to be picked up by other people with password.
seek emergancy medical advice and treatment if needed.
administer non-precription drugs
Applying sun creams

Are there anymore that any off you can think of, would be greatfully recieved if there are anymore:laughing:

I have permission to use;
plasters
anti-histamine cream/medicine
nappy cream
antiseptic cream
video camera
calpol
insect repellent
Saying; all the above must have first been tried at home and provided by parents.

plus all the usual!

The only other ones I have are using an assitant (may not apply) and giving record information sheet to emergency childminders.
