permission forms?

hi,I am just sorting through my paperwork and was wondering what permission forms I need to do ?????
My brain isn't working very well today :blush:
Up to now I have got:
Trampoline
Observations
Emergency medical treatment
Car
And that's as far as I have got has anyone got anymore suggestions please?:) x x x

if you are with the NCMA there are 2 permission forms in there covering quite a few things

sun lotion permission to apply if the child is unable to apply his/herself

permission to use a backup childminder in an emergency

permission to seek medical advice see NCMA emergency cards or similar from other sites
