Does anyone have a form for sharing information

Does anyone have a form for sharing information with other settings such as preschools, i have looked on bromley but cant find anything.

I use a daily diary to share with parents but need something i can use for preschool.

I have inspection tomorrow so am trying like mad to get everything i need up together.
