Hi - Can someone please let me know, on the NCMA accident forms the parent must sign to say that they have read,understood & recieved a copy of the form.

Baby mindee has bit her lip this morning toppling forwards, only its not her parents collecting this evening its granny. Can granny sign or shall I keep the form for parents tomorrow?

I will also write it up in the daily diary.

Not been in that situation but I would get granny to sign the form today and then when mum drops off/collects tomorrow I'd get her to countersign and date your copy to show she has been informed.

I'd also ring mum tonight to explain. Think that way you've covered all the bases

Good advice from Miffy.

Regardless of who picking up I always get a signature on form and make sure a copy with the childs diary.

I also (like Miffy) quickly call the parent just so that I have covered myself

I would get granny to sign, but contact mum as well and let her know, and write it in comm book ( though know you did this anyway). Then you could always add extra signiture tomorrow(with tomorrows date) but add that parent was made aware at time of incident by phone and granny signed. just document it all.

I take a photocopy too just incase the signed copy didnt make it into the hands of the parents. I have a child who's parents are seperated, their parents partners sometimes pick up but its double paperwork to ensure mum and dad are aware of the accident ... their communications skills are pants
