Bump on head......help

Hi All wondering if u all could give me abit of advise.....Pick my mindee up from after school club this evening and notice a bump on her head,ask my mindee what had happened and she told me, when i ask the after school club, they just said they didnt know nothing about it and that how it was left even though you could visably see she had bumped it recently......now what i need to know to cover myself do i just write out my own (record of excisting injuries)and get her mum to sign it.......Hope some could advise on this one..... record it as an existing injury - detail that you ask after school club and what they said etc....

I would get the parent to sign that you have told/shown them

To cover yourself I would make a note in your accident book that it happened at the club and get parent to sign.

ive got a form for existing injuries from other settings i fill this out give paretns one and keep one (and get them to sign it )
