Completely forgot but I have a parent coming to sign contracts etc in the morning for her 6mth old. I have record forms etc, but now ofsted are so hot on starting points I could do with something for mum to fill in ( she's a parent of an older holiday mindee too and I know she wont give me the info unless I wave a form in her face and make it look all official as she's just so chilled about me having her kids and couldnt give a monkeys about paperwork etc)
Does anyone have anything I could adapt as Im really short on time now and not sure how long I'll have to sit and do one of my own this evening ( DD2 is poorly :( )
