Daily diary

I write a daily diary on my 3 year old. I use a sheet that I fill in saying what he has eaten/activities/mood etc(also a few ticks here and there). But it's more based on a baby as it says nappies/sleeps etc.

Does anyone have a sheet I can use that would be geared more to the 3 year old? His mum does loves reading them and keeps them (I photocopy them and put them in his folder).
