From memory I think I put that I checked all aspects of my service to check for quality. I get regular verbal feedback from parents and before I completed my review I sent out questionnaires to parents and school age children. I then used the responses to these questionnaires to evaluate if my service could/should be changed in any way.

It is about 6 months since I completed mine but please don't worry about it too much. The form is VERY repetitive and not very well designed imho!!!
