what form do you use for eyfs, observations?

i just wondered if you use post it notes, or print off a sheet everytime you do an observation, or maybe you use a book. Im just thinking of the easiest way to log my observations, and i just needed some ideas on what format to use? sorry but im feeling a bit thick tonight!

I have a folder for each child.

In the folder I put a print out of the age and stage development stuff from Eyfs ...

Then I add a monthly observation sheet - 1 obs from each area of eyfs per month, with a next steps if appropriate

I tend to do one more in-depth obs per child per month... like a 'wow' moment

One (or more if we've been busy) photo obs per month

Copies of the children's colouring, mark making etc and photos of models or things that have gone home...

Over the year or so it takes to work through the age and stage Eyfs stuff, it builds into a lovely memory book :D
