Did anyone else find it stressful filling this out? I've had 2 babies in the last 5 years and I really cant remember minor illnesses inbetween, only that I had exhaustion a couple of months ago and I have gilberts syndrome.

Would depression include postnatal depression?

Would hospital admissions include the c-sections I've had?

It does go on a bit doesn't it?!?

You need to include anything that might be picked up when the form goes to the doctor.

That includes anything you've taken medication for like pnd.

Yes c-sections are hospital visits even planned ones... they are major abdominal surgery after all.

Ultimately the doc has to say if he thinks you are fit to do the job, so best be honest at this point.

I so stressed over mine, I even called them because I couldn't remember that far back and I didn't want them to think that I was hiding anything from them. I had depression and couldn't remember the dates i'd had off work with it and they said I could just put various, as long as I'd put it down.
