I am wanting to put together a simplified version of the development stages or I think some pre-schools used to have stepping stones - way back when I used to take my dd to pre-school they used to have a caterpillar with the different stages for her age range.

I would like to do something like this to use with parents. Something that they can become involved in and show input coming from them about their child at home as well as information I provide.

Has anybody got any ideas or has something like the caterpillar - I particularly want to make it really easy to understand and simple to complete. If it is quick then there is a good chance that parents will actually do it. Worth a try.

Any help would be appreciated

I've not seen anything like this for eyfs tbh...

Have you looked on TES? There are some unusual things on there sometimes :D
