template of invoice

anyone got an example of an invoice they give to parents, i have a contract for 40hrs a month which is paid upfront but usually there will be surplus hours and I need to invoice these hours at the end of the month. Just wondering what to include and to keep it simple.

I have an invoice I use monthly but it is not Excel - just one you can print and write on. I'm not so clever on Excel. Anyone is welcome to a copy though.
